A separate scholarship application – in addition to the university admission – is needed for Tuition Fee Waiver. You’ll have to collect these requirements:

  • Form A (Checklist for Submission)
  • Form B-1 (Application Form)
  • Form C-1 and C-2 (List of Household Incomes/Deductions)
  • Form D-1 (Submission of Certificate of Residence)
  • Form D-2 (Submission of Documents to Certify Financially Independent Applicant)
  • Form E (Tax Exemption Certificate)
  • Form F (Employment Income Statement)
  • Form G (Own Business Income Statement)
  • Form H (Temporary Income Statement)
  • Form I (Student Declaration of Part-Time Job)
  • Form J (Status of Other National School Student than the Applicant)
  • Form K-1, K-2, K-3, K-4, K-5 (Special Deduction Statement)
  • Form K-4 (Medical Expense Statement)
  • Form M (Recommendation from Advisor of Student with Special Circumstances)
  • Copy of Bank Passbook
